Mechanism of Action

  • Metformin (Biguanide):

    • Decreases glucose production in the liver

    • Reduces intestinal absorption of glucose

    • Improves insulin sensitivity and peripheral glucose uptake

  • Teneligliptin (DPP-4 inhibitor):

    • Inhibits DPP-4 enzyme, prolonging action of incretin hormones (GLP-1, GIP)

    • Increases insulin secretion & decreases glucagon release in a glucose-dependent manner

  • Combined Effect: Provides effective control of both fasting and postprandial blood sugar levels

Dosage & Administration

  • Usual Adult Dose: 1 tablet once daily (or as prescribed by physician)

  • To be taken with meals to minimize gastric side effects

  • Swallow whole with water – do not crush or chew

  • Dose adjustments may be needed depending on blood glucose monitoring, kidney function, and other medications

Side Effects

  • Common: nausea, diarrhea, abdominal discomfort, headache, mild hypoglycemia (especially if combined with other agents)

  • Rare: skin rash, dizziness, taste disturbances

  • Serious (rare):

    • Lactic acidosis (risk with metformin, especially in kidney impairment, alcohol use, or heart failure)

    • Pancreatitis (rare with DPP-4 inhibitors)

Precautions & Warnings

  • Use strictly under doctor’s prescription

  • Avoid in patients with severe kidney or liver disease, heart failure, or history of lactic acidosis

  • Not for use in Type 1 diabetes or diabetic ketoacidosis

  • Regular monitoring of blood sugar, HbA1c, and kidney function is recommended

  • Caution with alcohol (increases lactic acidosis risk)

  • Pregnant & breastfeeding women: use only if clearly prescribed by doctor
